In this position you will work with the ward team to reduce inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting.

To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making and to take responsibility for the daily therapy input and support of delegated patients.


You must be able to work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role, working within a multi-professional team undertaking practitioner role.

Implementing treatment and care to patients under the guidance and supervision of a registered professional.

  • To work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role
  • Wherever possible to contribute to reduce inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting
  • To practice the principles and philosophy of individualised personcentred patient care in accordance with Trust Policy, procedures and regulatory body's guidelines
  • To work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role
  • Deliver treatment/care on a ward setting, under the guidance of a registered practitioner
  • To take responsibility for the daily therapy input and support of delegated patients
  • To work within a multiprofessional team undertaking practitioner role
  • To work with colleagues on the ward and other organisations including voluntary sector and Social care
  • To take responsibility for the daily care and support of delegated patients
  • To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making


Our team is based at Totnes Hospital and provides rehabilitation and discharge planning to the patients on the community hospital inpatient ward.

The multi
- disciplinary community team have a hard-working, flexible and can-do approach. The team are a supportive and friendly, adapting to the ever-changing demands on the service.


You will be joining a multi-disciplinary team which aims to provide rehabilitation to patients staying on the community hospital ward.

You will work as part of the Therapy team to plan and arrange safe and effective discharges home and to support with identifying care needs and ongoing rehabilitation needs for people.

You will also carry out rehabilitation programmes and support with lunch clubs/exercises groups on the ward. The Therapy team consists of an Occupational Therapist, Physiotherapist and a support worker.
