This is an exciting opportunity to work within a team of Community Nurses in Holsworthy the role will include working clinically to offer expert management of patient complexities and day to day operational management and leadership to a team of Community Nurses. To be responsible and accountable for the team caseload. To provide assessment and nursing care, advice and information to patients and carers within the home environment or alternative care setting.

To provide clinical and operational leadership to a team of nurses and support workers so that they, in turn can provide safe, effective and compassionate care for patients and their families in Holsworthy and surrounding areas.

Working Pattern: hours per week over 3 days.

Interview Date: Friday st June 4.
