The Health Care Assistant Works under indirect supervision of the Senior Practice Nurse. Is accountable to the Senior Practice Nurse and the Partnership/Leadership Team for their actions in undertaking delegated work.

Provides support for other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole.

Refers to more senior members of the Primary Health Care team as appropriate.

Works within all relevant practice policies and procedural guidelines for example: infection control, chaperoning.

Is required to keep competencies up to date and partake in ongoing training and appraisal process for continuing professional development.

Promotes effective communication and relationships within the team. Works collaboratively as a team member to achieve the aspirations and objectives of the Practice as a whole.

Actively participates in clinical and practice meetings. Contributes to the development, monitoring and evaluation of care delivery.

The Health Care Assistant undertakes agreed procedures which are likely to include:

Venepuncture BP checks, height, weight, BMI, urinalysis, ECG NHS health checks Chronic disease health checks Spirometry Assisting in minor procedures/minor surgery Chaperoning other clinical staff Supporting patients who are anxious or distressed Immunisations eg seasonal influenza.

The Health Care Assistant undertakes to support patients in their physical, mental and social wellbeing by:
Encouraging and supporting patients to address issues which affect their health and social well-being.

Engaging in opportunistic or formal Health Promotion where appropriate. Working with the Practice to support vulnerable patients.

Communicating or escalating concerns to a senior member of the team in a timely manner Confidentiality In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers.

They may also have access to information relating to the practice as a business organisation.

They will manage their own and others health and safety and infection control as defined in the practice Health and Safety policy, the practice Health and Safety manual, the practice Infection Control policy and other published procedures.

This will include (but will not be limited to):

Using personal security systems within the workplace according to practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general patient areas clean, sterile; identify issues and hazards or risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business Equality and diversity: The post-holder will support the equality, diversity and rights of patients, carers and colleagues, to include: Acting in a way that recognizes the importance of peoples rights, interpreting them in a way that is consistent with practice procedures and policies, and current legislation Respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues Behaving in a manner which is welcoming to and of the individual, is non-judgmental and respects their circumstances, feelings priorities and rights.
