Job summary

An exciting new opportunity has arisen for a highly motivated, experienced, vibrant nurse to join the Palliative Care Team at Florence Nightingale Hospice working in the Day Hospice caring for individuals with life-limiting conditions.

Working as part of an enthusiastic, committed team the role will require you to provide patient centred care to individuals with Specialist Palliative Care needs. The Day Hospice role includes managing and encouraging group dynamics, providing psychological support, and using excellent communication skills to enable Advanced Care Planning and End of Life Care discussions.

You will be required to use your nursing experience to provide the highest standards of nursing care, working as part of an established MDT.

Main duties of the job

To provide clinical input to Day Hospice.

To undertake complex assessment of patients' needs

To plan, monitor, evaluate and review the patient's planned programme of care along with the multidisciplinary team.

In the absence of Day Hospice deputy team leader, you will share day-to-day management and supervision of Day Hospice staff and volunteers.
