MAIN DUTIES/RESPONSIBILITIES 5.1Responsibility for People Management Competencies and Training: To be involved in the planning, implementation, delivery and evaluation of clinical and essential training using relevant national and local guidelines and frameworks.

Support in delivery of training programmes led by other clinical educators.

Implement, participate, and facilitate in the development of Practice Supervision as peer groups for all staff groups across the Community Nursing teams.

Facilitate the development of a peer learning groups for all staff groups across the Community Nursing teams.

Develop opportunities to enhance development and sign off, of clinical practice competencies. Facilitate and support in the sign off, of clinical competencies, supporting evidence based, quality leadership. Support with the delivery and implementation of essential clinical training for these staff groups. Provide expert clinical leadership and support to all staff groups across the community Nursing teams.


Student Nursing Associates and nurses acting as a role model in provision of clear direction for nursing and practice development.

The role includes a balance of clinical, educational and leadership responsibilities which may vary according to need and to be agreed by the Practice lead for nursing to support service development.

5.2Responsibility for financial and/or physical resources Ensure training equipment and consumables are maintained.

Monitor stock levels and liaise with training team in regards ordering and restocking Maybe required to instigate ordering of new or updated equipment in line with current guidelines and best practice responsible for the safe use of equipment other than equipment which they personally use; And/or responsible for maintaining stock control and/or security of stock.


5.3Responsibility for Workforce and Operational Issues: Works within the NMC Code of Practice. Ensure quality service delivery by developing and reviewing of workforce competencies in collaboration with professional training and development, practice lead for nursing and operational mangers. Support on strategies to support Professional training and development and Practice lead in the development of suitably skilled staff by using workforce planning developments to support the development of sill mix competency and development within the staff groups identified. To actively participate in supporting new ways of working and developing staff quality and competence within clinical practice.


Develop opportunities to enhance development and sign off, of clinical practice competencies Remain updated and competent and ensure that clinical practice is evidence based.

Have an up-to-date personal development plan and professional portfolio Participate in an annual appraisal of their work in line with the Knowledge and Skills Framework (KSF) where the job description will be reviewed, and objectives set.

In line with the annual development plan the post holder will be expected to undertake any training or development required to fulfil their role.

5.4Responsibility for people who use our services Provide nursing knowledge and experience to any member of the multi-disciplinary team in relation to patient care, ensuring a visible presence is maintained within clinical areas.


Set, agree and monitor standards for clinical practice and patient care and ensure teams have the right skills to deliver a high-quality service.

Ensure changes in clinical practice are reflected in training and competency assessment, alerting the team manager and practice lead for nursing where there is an unmet need.

Work autonomously in the community setting, recognising the risks associated with lone working, e.g., aggressive behaviour, and work proactively to reduce those risks.

Work as an autonomous practitioner, managing and prioritising own workload according to local guidelines and identified need.


Working in a variety of community settings, exercising a high level of judgement, discretion and decision making in clinical care.

To facilitate and support effective clinical assessment and transfer those skills to support quality of care and clinical competence for those identified staff in line with clinical assessments listed below:

To Support and perform a comprehensive holistic assessment and physical examination for patients using clinical skills and knowledge to identify problems or symptoms.

To be able to determine if symptoms warrant further investigation and/or treatment and initiate this as appropriate. To plan, implement and evaluate on an ongoing basis, care delivery using evidence based best practice.

To be able to identify and resolve complex clinical issues.

Support staff in providing expert clinical care and health promotion interventions that will help prevent unnecessary hospital admissions and periods of crisis.
