An exciting opportunity has arisen to work within the Cardiology Team as Advanced Clinical Practitioner.


The post-holders will exercise advanced clinical skills, levels of judgement, discretion and decision-making in cardiology care, demonstrated through, and supported by, professional portfolio evidence.


You will work collaboratively with the multi-disciplinary cardiology team and allied specialties to ensure efficiency and effectiveness in the provision of accessible, high-quality cardiology care at Warwick Hospital.


Clinical duties will include a combination of shifts across the Cardiology Unit, including ward rounds and working alongside the cardiology on-call team, including participation in the on call cardiology team rota.

Duties will include, but not limited to, clerking and review of patients in the Emergency department (ED), review of patients on non-cardiology wards, independent general cardiology and subspecialty clinics, providing support and advice across the MDT and contributing to the multidisciplinary teaching agenda.

Regular evening work and weekend working is required.


During winter months and at other times of high demand, the post-holder will provide medical care to and expedite discharge of patients in our 'flex-capacity' beds.


Approximately 20% of the role will be designated to the ongoing development of innovative practice, service improvement and standards of care.


You will have a critical role in ongoing development of cardiology services and new processes to improve patient flow and rapid streaming of cardiology referrals to the most appropriate assessment or treatment area.


The post-holders will be expected to contribute significantly to clinical governance, audit, research, training and education, providing leadership to the cardiology team in one or more of these areas.

Support development and implementation of personalised care pathways for the speciality through holistic needs assessment, planning and evaluation of care and all care interventions.


Implement pathways of care, adapting to patients' emerging needs by ensuring a planned and co-ordinated approach to care, supporting enhanced recovery and reducing inappropriate admissions to improve patient experience and safety.


Refer directly to other health care professionals and specialist departments as necessary, to ensure optimal efficiency and quality of care.


Act as a patient advocate to facilitate the process of shared decision-making in respect to health, choice of treatment and care.
