Job Summary

An amazing opportunity is available for a full-time Clinical Nurse Specialist to join the Community Paediatric Team within Ealing Services for Children with Additional Needs as part of Ealing Community Partners. This role is also suitable for an experienced Health Visitor interested in transitioning into the Clinical Nurse Specialist role.

This role aims to expand the existing specialist health visitor roles within the Ealing Child Development Team, transitioning them into Clinical Nurse Specialists with a focus on working closely with Community Paediatricians to provide top-notch, patient-centered care.

The selected candidate will be stationed at Carmelita House, collaborating with Consultant Paediatricians and other health professionals to support children and families. The role also involves responsibilities at various locations across Ealing, requiring the ability to travel around the borough.

Main Duties of the Job

We are searching for a nurse with experience in caring for children and young people with complex developmental needs, capable of delivering high-quality care to children impacted by their disability alongside their families. Responsibilities include conducting clinical assessments, arranging investigations, making referrals, contributing to assessments, providing sleep training and counseling, collaborating with other teams, attending multi-agency meetings, and serving as a resource for parents and colleagues seeking guidance on complex children. The ideal candidate should have prior experience working with children with special needs, possess strong teamwork skills, demonstrate flexibility and innovation in healthcare, work autonomously, and prioritize children's well-being.

Specific training in clinical assessments will be provided as necessary, with opportunities to acquire nurse-prescribing qualifications to cater to the needs of children, particularly within the sleep service.

About Us

Ealing Community Partners (ECP) is a collaborative provider managing Ealing community health services under Ealing CCG. Led by West London NHS Trust and in partnership with Central North West London, ECP is responsible for employing the successful candidate.

Ealing's community pediatric services, located at Carmelita House, house Ealing Services for Children with Additional Needs (ESCAN), a joint Health and Local Authority initiative hosting specialist community children's services, children's mental health services, educational services, and a social work team for children with disabilities. Clinics are conducted at various locations in Ealing, including specialist schools and nurseries.

Located in West London, Ealing is a dynamic residential suburb and the fourth largest borough in North West London, with a population of around 340,000, including 67,000 children aged 0-15. The borough boasts a significant migrant population, with approximately 40% of residents born outside the UK.

Supervision and adequate training will be provided to ensure you stay updated and equipped for your role.

The ideal candidate should be prepared to work full-time Monday to Friday, 9-5, with potential for job sharing or part-time arrangements. Flexibility in working hours can be discussed with the clinical lead.

Job Responsibilities

Main duties and responsibilities:

  • Provide support and resources for families, professionals within ESCAN, and other agencies as a central contact for the Ealing Community Paediatric Team.
  • Conduct comprehensive health needs assessments of children and young people with complex needs within the Child Development Service.
  • Lead the Sleep Pathway for children and young people requiring specific support.
  • Lead the Down Syndrome pathway.
  • Collaborate in multi-disciplinary teams to support Consultants with specialized assessment pathways.
  • Contribute reports as needed for the Education and Health Care Plan process.
  • Participate in relevant meetings, including safeguarding, child protection, and specialist pathway meetings.
  • Coordinate care for children with life limiting conditions and their families as directed by the consultant or Service Lead.

Qualifications and Training

Essential

  • Community Specialist Practitioner qualification
  • Registered Nurse Learning Disability (RNLD) or Registered Nurse Child (RNChild)
  • Evidence of Post-registration education at degree level

Desirable

  • Nurse Prescriber Specialist Course completion

Experience and Knowledge

Essential

  • Significant post-registration community experience
  • Experience in key working and care coordination
  • Experience in collaborating with professionals and agencies, staying current with clinical practice and health legislation
  • Proficiency in child protection
