Overview:

The Discharge to Assess Team is a busy point of referrals for adults, awaiting assessments and reviews at home or stepdown beds under the Discharge to Assess process and the Rehab Wards which is currently being used as a step down facility forclients being discharged from hospital.


Requirements of the role:
Registered with Social Work England
Minimum 2 years of experience in assessing Adults and procedures and similar services

Assessing clients in stepdown beds, 24 hour care packages when they are discharged home for long term packages of care - D2A pathway.

Visiting clients in their home environment or step down placements
Knowledge of the Care Act, Mental Capacity Act, NHS Continuing Healthcare, Safeguarding and other relevant legislation for these client groups
Identify and carry out Mental Capacity Assessments
Skilled report writing and manage the recording and administration around assessment and planning and agreeing care provision through up to date, thorough and accurate recording on relevant IT systems as required
Office based and hybrid working is allowed now but adhering to regulations.
Arranging reablement or occupational therapy intervention as well as sign posting to other resources and offering information and advice.
