We are looking for a highly motivated and autonomous person to lead a multi-disciplinary team which will take forward the 2 Hour Urgent Community Response for Lincolnshire.

This service provides holistic assessments, treatment and support to patients in their usual place of residence who are experiencing a health or social care crisis who might otherwise be admitted to hospital.

You will lead a service in which care is provided by a multi-skilled team including nurses, paramedics, social care, occupational therapists and physiotherapists.

This role is as part of the MDT for Lincolnshire's Urgent Community Response Team.

You will actively lead and continuously develop this service to ensure it supports patients in Lincolnshire both virtually and physically to respond to a form of urgent crisis and avoid admission.


With extensive experience in community rehabilitation you will be performance and outcome focussed with a passion and drive for making this new service a success through effective and compassionate leadership.

You will need to possess the ability to work across a variety of disciplines within Lincolnshire including partnering healthcare providers, social care and the community voluntary sector therefore the ability to foster positive and productive working relationships is key.

The role a countywide role across Lincolnshire but will be predominately based at Beech House, Lincoln.
