To be a valued member of the Multi-Disciplinary Team, supporting and monitoring the well-being of service users under the guidance of Care Coordinators. Delivering holistic care to empower service users towards their recovery goals.

Main Duties of the Job

  • Active involvement in direct service user care within the community
  • Ensuring a service user-centered multi-disciplinary approach
  • Recording need assessments and risk assessments
  • Adhering to Trust policies and registered nurses' instructions
  • Collaborating with healthcare professionals to provide meaningful activities
  • Promoting anti-discriminatory practice
  • Supporting Physical Health, Depot, and Clozapine clinics
