An exciting opportunity has arisen to join our Inpatient department as a Patient Flow Co-Ordinator.

Your role will involve working along side nursing and therapy staff to facilitate the admission in to and discharge of patients from our wards in an administrator capacity.

The role involves both office based and patient facing elements.

Good IT and communication skills are essential and prior experience within a care setting beneficial. You will require certification to level 3 or 4 NVQ in admin support or an equivalent.

You will work among a dedicated team to deliver high quality individualised plans of care in preparation for a safe discharge.


You will work closely with our In reach team based in the Royal Berkshire Hospital communicating vital information on safe transfers of care.

You will be based in West Berkshire Community Hospital working alongside nursing, therapy and medical teams.


You will liaise directly with patients and carers to ensure that patients are assisted in a safe and timely discharge.
