We are lookingfor a frailty nurse to join our innovative Ageing Well Team across the PCN.

This is anexciting opportunity for an enthusiastic, motivated Registered Nurse, workingclosely with our O/T, physio, frailty assistant & care coordinators toprovide proactive care for the moderately frail. This role is integral to theassessment of patient frailty levels & assisting with review of patients toallow the best access to care required for their support. You will be anautonomous practitioner who is adaptable, has a flexible can-do attitude with apassion for excellence & ability to work well both on your own & aspart of a team.

This is acommunity-based role, applicants must have a full driving license & accessto a vehicle daily.

Main duties of the job

The successful candidateshould be an experienced practitioner who willwork in partnership with other health professionals in supporting frailpatients and to provide holistic patient care. They will act within theirprofessional boundaries and competencies, assessing, planning &implementing patient care.

They will participate inMDT meetings to discuss patients that the Ageing Well Team are supporting. Asa result they must have a high levelcommunication/interpersonal skills to negotiate & establish collaborativepartnerships with patients & care professionals and have enthusiasm &the ability to use youre their own initiative.

About us

Theaward-winning Selby Town PCN consists of four practices with a combinedpopulation of approximately 50,000 patients, covering the large geographicalarea surrounding Selby and Escrick.

The PCNworks over four practice sites (Beech Tree Surgery, Escrick Surgery,Posterngate Surgery and Scott Road Medical Centre) our local community hospital& The Summit, an ex-indoor adventure park. The PCN has a Clinical Director,two Clinical Lead GPs and is managed by the PCN Operations Manager. We work inincreasingly collaborative partnerships across a number of workstreams, whichinclude important priority areas of mental health, frailty, urgent care andhealth inequity. Our team consists of a diverse multi-professional workforcewhich includes pharmacists, MSK practitioners, mental health workers, carecoordinators, social prescribing link workers, a frailty therapy team, anurgent care coordinator, ANPs, paramedics, ACPs, a transformational lead, adigital lead, a dietitian, a health & wellbeing coach & an HR lead.

Job responsibilities

KEYRESPONSIBILTIES

To visit clients who meet the Frailty Nursingcriteria within their usual place of residence.

To assess and identify the health needs ofpatients who are frail and who may have a long-term condition.

To assess, plan, implement and evaluateprogrammes of care to address the identified needs of patients identified asbeing frail and/or those with long-term conditions.

To support the development of patientsindividual Personal Health and Social Care Plans.

Develop disease specific patient informationand education to support patients self care.

Participate in education & staff development, actively participatein regular supervision.
