What does the role involve:

  • Carrying out full risk assessments and a personcentred outcomebased care plan for each new client
  • Carrying out care visits as part of the position
  • Complete quality monitoring visits with your clients
  • Completing regular care plan reviews
  • Completing regular MAR chart checks, both on eMAR and paper MAR charts.
  • Rostered paid "On Call" duties, ensuring that our service runs smoothly during out of office hours and that all communications are documented and shared internally
  • Liaising with other Healthcare professionals such as Social Workers, Occupational Therapists, GP Surgeries etc
  • Coaching and mentoring new care staff

Our Requirements:

  • You must be a caring and compassionate person who is looking to deliver outstanding care and work within the team
  • You must be willing to travel around Waveney
  • You must the ability to deal with problems and final solutions, and be able to work with a pressured environment ensuring that the team is competent at all times.
  • You must hold a valid UK driving licence and have you own transport
  • NVQ level 3 would be desirable but not necessary but you would have to complete whilst on the job
