Job summary

An exciting opportunity has arisen to wok within a busy integrated community respiratory service to cover 12 months maternity leave as part of the pulmonary rehabilitation team. The post is to support the team in the provision of pulmonary rehab groups for people with chronic respiratory disease. The post is for Monday - Friday.

Main duties of the job

o To deliver care and rehabilitation to groups and individual patients with the support of more senior clinicians, to meet the patients' individual goals within the community To undertake "phase 2" telephone calls to all referrals received by the service to help increase acceptance rates onto the To gain informed patient To be able to recognise signs and symptoms of the deteriorating respiratory To immediately inform more senior members of staff of any adverse reaction to exercise and to complete the appropriate incident To be able to communicate with patients, family and carers who may have barriers to understanding , mental health problems or deafness, to maximise their rehabilitation To have good verbal and non- verbal communication skills of motivation and encouragement to help motivate To help maintain equipment to a good standard, including regular cleaning, servicing, and highlighting to the team leader when new equipment is To be able to teach patients to use rehabilitation equipment safely and To be an active member in the delivery and attendance of the in-service training To provide spontaneous and planned advice to patients, family and carers related to their diagnosis and To support the delivery of pulmonary rehabilitation classes.

Job description

Job responsibilities

To be responsible for the management of own workload. To promote the role of pulmonary rehabilitation to students and other health care professionals. Contribute to discussions on service delivery and development. To help in the monitoring, storing, cleaning and servicing of equipment used by the pulmonary rehabilitation group and nebuliser machines. To maintain equipment competencies used within the team and adhere to departmental usage. To maintain accurate, comprehensive and timely clinical and non-clinical documentation, in line with legal and departmental requirements. To be actively involved in the collection of statistical data used by the department. To be aware of health and safety aspects of working in the community, including lone working and recording of incidents, and ensure all equipment is safe for use. To comply with departmental policies and procedures. To undertake any other duties that might be considered appropriate by the team leader after adequate training. Ensure all accidents, incidents and hazards are reported and dealt with according to Trust requirements. To comply with the Data Protection Act and Caldicott recommendations.

To maintain accurate documentation in line with local policies and procedures.

Qualificantion and Education

Essential

NVQ 3 in health and social care / therapeutic support or relvant equiviant Evidence of formal education with core numeracy and literacy skills Commitment to formal education

Desirable

Basic IT skills Motivational interviewing training

Essential

Be able to assist, implement and modify treatment programmes Prioritisation skills to manage own workload independently. The ability to understand and respect patients and carers differing points of views.

Expeirince and Kowledge

Previous experience as HCA or rehabilitation/therapy assistant Evidence of independently managing own caseload Liaising with other agencies / professions

Community expereince Working with patients with chronic respiratory disease Working with palliative patients
