Job summary
This is an island-wide role, working across the Isle of Wight's three Primary Care Networks and the NHS Trust. The post holder will work closely with the VCSE Transitions Lead to ensure the creation of seamless pathways for young people aged 16 to 25.
The 16 to 25 MH Transitions Co-ordinator will proactively work with services for young people between the age of 16 and 25 who are actively transitioning between children's and adult's mental health services across the system, to ensure appropriate coordination and navigation of care and support across health and care services. They will play a key role in ensuring individuals get to the right services at the right time, providing a point of contact and access to appropriate levels of care.
Main duties of the job
Care co-ordinators review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.
This role will form an integral part of the 16 to 25 CYP Transitions multidisciplinary team at CAMHS, as well as working into GP practices, liaising with social prescribing link workers and health and wellbeing coaches, and working closely with our third sector partners to promote and embed a personalised care approach across the wider system.
Key responsibilities of the 16 to 25 MH Transitions Co-ordinator will include:
Working with young people, their families and carers and the team to improve their experience of transitioning to adult services and supporting them to access personalised care and support to manage their needs and achieve better healthcare outcomes.
Helping people to manage their needs through answering queries, making and managing appointments with appropriate services, liaising with system partners, and ensuring that people have good quality written or verbal information to help them make choices about their care.
Assisting people to access self-management education courses, peer support, the 16 to 25 Wellbeing service, and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills and confidence in managing their health.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Essential
Educated to a minimum of GCSE/O Level standard or equivalent, including English Language and Maths.Desirable
NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards.Experience

Experience of working in a health or care setting in direct contact with people, families or carers (paid or voluntary). Knowledge of how the NHS works, including primary and secondary care, and how the wider system functions. Understanding of the needs of people with a long-term mental health condition, particularly in relation to promoting their independence.Desirable
Lived experience of managing mental ill health, or of caring for someone with mental ill health. Experience and / or training in Peer Support Work. Experience of working within multi-professional team environments. Experience or training in personalised care and support planning. Experience of data collection and using tools to measure the impact of services. Knowledge of Safeguarding Children and Vulnerable Adults policies and processes. Experience of working with vulnerable people, complying with best practice and relevant legislation. Basic knowledge of long-term conditions and the complexities involved.-
