Accurately record and action telephone messages from patients, relatives, and other agencies in a timely manner
Deal with Information Governance requests as required
Communicate with all persons in a professional, polite, and respectful manner, in accordance with local and Trust policy. V- 2 October 2020
Coordinate appointments as requested and collect record and submit statistics on a regular basis: -Key performance indicators -MES surveys
Undertake and understand the 'buddying' process
Identify and use the most effective means of communication appropriate to the situation and individuals involved, where there may be barriers to understanding for example anxious patients, cultural differences, language, or communication difficulties
To observe confidentiality of all information, adhering to the legal requirements of the Data Protection Act and Trust information sharing policies
Be responsible for incoming calls to the service, prioritising and dealing with enquiries, taking detailed messages, and responding or referring to the relevant team.
Ensure courteous, efficient and robust lines of communication exist between patients and the locality team
Co-ordinate the delivery of key administrative functions related to patient flow
Organising meetings and providing meeting administration, taking notes as agreed, actions and ensuring prompt distribution and dissemination in a timely way
Liaise and communicate with referrers, service users, relatives, carers and other health and social care professionals to support and co-ordinate care
Insure all those involved in the care process and subsequent co-ordination are kept informed of progress.
Ensure case notes have been documented and recorded locally and case closed
Participate in departmental meetings, including taking minutes, and to ensure effective liaison with all colleagues internally and externally
