Are you an enthusiastic and dedicated Advance ClinicalPractitioner and a registered Nurse or AHP or Pharmacist?

This is an exciting opportunity for an Advanced Clinical Practitioner to join our Guide Bridge Medical Practice team on a fixed term basis to provide care for patients in the Audenshaw locality.

Guide Bridge uses a digital front door which means that all patients will be assessed by a clinician prior to having an appointment. This means improved patient satisfaction, a safer and better service for patients and the patients see the right clinician/service at the right time.

Our Practice works closely with Denton, Audenshaw and Droylsden PCN and offers access to mental health workers, social prescribers, MSK first contact practitioners, nurse associates, pharmacists and other ARRS roles within the PCN. As the practice is part of GTD healthcare, this enables the you to have access to specialist support such as gtd peadiatric leads, gtd learning disability leads and gtd palliative care leads. Salary: £49000 to £65250 a year depending on experience

Part time and bank positions available

Working patterns required: Friday between 8am - 6:30pm

Main duties of the job

Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required).

You will be supported by the non clinical team who have a good knowledge of our extensive local hospital and community services.

The clinical team includes GP's, Practice Nurse, Pharmacist, Paramedic and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.

In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.

You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP.

Job responsibilities

Clinical Practice

Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis. Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Refer and discharge safely ensuring that safety-netting advice is offered. Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Maintain effective communication within the organisation and with external stakeholders. Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Ensure professional revalidation requirements are met. Deliver care according to NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate. Support and participate in shared learning across the organisation and wider organisation. Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.

Registered first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.

Desirable

Mentor/Teaching qualification. Clinical supervision training and experience.
