Job Description:

Purpose of the role:
To support key activities across the PCN; supporting all PCN practices by co ordinating activity and providing an efficient, well organised operational lead role to the Care
Coordinators to support them in their roles and ensure effective timely delivery of the PCN objectives and contractual requirements.

To ensure equity of service across the PCN and manage steady continual output from the
Care Coordinators to progress attainment of KPIs.


The Senior Care Coordinator will also be part of wider discussions and decision making with the Chief Operating Officer, Clinical Director and PCN Board and will be the accountable manager for all Care Coordinator roles.

This will include management record keeping as well as working with the COO on performance management, recruitment, training and development, appraisals, DES and other contractual compliance.


Key activities and responsibilities:

  • Managing day to day rotas and work schedules of Care Coordinator staff, including managing annual leave, training and sick leave and liaising with the
Practice Managers re room allocations

  • Ensuring equity of workload and access for the PCN practices to each specialty of care co ordination
  • Have a thorough working knowledge of each of the Care Coordinator roles in order to train, assess, audit and improve systems and individual output
  • Have a good working knowledge of contractual requirements and be part of discussions with the COO and CD on operational delivery to achieve IIF/DES requirements
  • Have strong system working knowledge of all MS Office packages - in particular excel and the ability to train and demonstrate clinical and Data reporting systems to team members.
  • Monitor IIF and DES achievement and support the coordinators with workload and planning strategies to timely achieve.
  • Have the ability to support the Care Coordinators by having practical involvement in assisting with their workload at deadline critical times or times of absence
  • Work with the Project Support Officer to produce job descriptions and adverts for
Care Coordinator roles including the management of shortlisting and interviewing for appropriate positions as delegated by the COO

  • Work under the supervision of the COO, alongside the Project Support Officer to ensure attainment of all HR processes including pre-employment checks, new starter documentation, and induction programmes, occupational health screenings, ongoing Sickness recording, appraisals, mandatory training etc.
  • Responsibility for ordering of vaccine and related consumables; including regular submitted stock takes and wastage reports and distribution of vaccine to practices.
  • Manage and maintain a central electronic documentation storage system requiring the scanning and filing of all documents for all staff reporting to the role
  • Distribution of central communications to Care Coordinator team including training and wider networking opportunities
  • Lead team catch ups, Teams meetings and face to face regular meetings
  • Lead 1:1 regular sessions with individuals, reporting back to COO progress of individuals and any matters for discussion
  • General administrative duties relating to the role or wider PCN
  • It should be noted that whilst this job description lists the main areas of responsibility, there may be additional tasks appropriately assigned by either by the COO or the CD to this role.


It is necessary to be a car driver and have access to a car to enable travel between practices, PCN estates and event venues.
