Role Summary

This is a temporary position with the Breast team until early December. You will be part of a team of professionals assisting Clinical Nurse Specialists in managing and supporting individuals on a Breast Cancer Pathway, from pre-diagnosis to post-diagnosis care. Your role is crucial in providing support and guidance to patients, identifying and addressing any potential delays in their care journey.

About the Role

  • Navigate, coordinate, and support patients throughout their care pathways.
  • Assist in arranging tests and appointments for timely diagnosis and management.
  • Provide telephone support to patients and triage calls and emails.
  • Organize referrals to other services, maintain patient records, and support data collection for audits.
  • Develop skills to differentiate between patient needs that require complex care and those that can be managed by others.
  • Engage in improving current practices and preparing for future service enhancements.
  • Offer psychological support to patients and participate in health promotion activities.
  • Contribute to the organization of support groups and health events.
  • Facilitate communication with patients, families, and other healthcare providers.

Working Hours

Approximately 24 hours per week spread over 3 days, with varying shifts based on service requirements.
