Pay Rate:
£41 per hour
Key Responsibilities:
Provide comprehensive social work support to children and young people with disabilities, ensuring their needs are met effectively.
Work collaboratively with citizens, carers, and families to develop personalized support plans aimed at promoting resilience and independence.
Advocate for the rights and preferences of the individuals, ensuring they are actively involved in decision-making processes.
Maintain accurate and up-to-date records of interventions, assessments, and outcomes.
Skills and Qualifications Required:
Recognized relevant social work qualification.
Minimum of 3 years post-qualification experience in a similar role.
Registration with Social Care Wales.
Excellent verbal and written communication skills.
Strong interpersonal skills with the ability to build rapport and establish effective working relationships with clients and colleagues.
Additional Information:
This is a short-term vacancy with the possibility of extension.
Working hours:

This role may require occasional travel within the Conwy area.
Competitive pay rate of £41 per hour.
