Psychosocial Substance Misuse Caseload Specialist

Job Overview
A Band physiotherapist with neurological experience is sought for a post within the paediatric and adult learning disability transition specialty.
The successful candidate will work in a supportive environment to acquire and develop knowledge and skills related to treating children and young people with disabilities in the community/school environment and supporting their transition into adult services.
A willingness and desire to learn are essential for this post, and the candidate will be supported by senior members of the team/clinical specialist.
Main Duties of the Job
With support from senior colleagues, the successful candidate will manage their own caseload in the community, predominantly covering schools, homes, day centres, and sports centres in Conwy.
They will also provide cover on the acute paediatric wards at Glan Clwyd Hospital and contribute to community work in Denbighshire.
The candidate will work one day per week supporting children and young people up to the age of 18 who have a learning disability during their transition to the Community Adult Learning Disability team.
They will contribute to the supervision of support staff and students and participate as a member of the wider multidisciplinary/joint agency teams supporting children and young people with disabilities.
