Job summary

The post holder will be supporting flow and patient pathways within the General Medicine Care Group. This will focus on flow from the Emergency Department to other areas of the hospital.

Main duties of the job

The post holder will be a clinically credible nurse, with the ability to apply critical decision making to influence patient care

The role will focus on patient flow across the care group; particularly focused on developing and implementing nurse-initiated care, such as criteria led discharge and SDEC pathways.

The post holder should have experience in developing and reviewing patient pathways to support effective care.

Experience in medicine management initiatives such as Patient Group Directive

Job responsibilities

1 Clinical

Ensure high standards of patient care at all times, challenging and ensuring others address poor practice, in accordance with the NMC Professional Code

In collaboration with senior members of the nursing team, provide assurance that the standards set out in the relevant Nursing & Midwifery strategy and direction of Nursing & Midwifery for the Trust is delivered at ward level.

Ensure local plans support all clinical quality external accreditation, for example Care Quality Commission (CQC) essential standards, Commissioning for Quality and Innovation (CQUIN) and Quality Accounts.

Ensure agreed systems and processes are in place to consistently provide safe, effective high quality patient care and embed a culture of improvement, focusing on the Trusts Harm free indicators, measuring practice and using variations in the data to develop local improvement projects on the ward.

Through visible clinical leadership ensure that the delivery of kind, compassionate and respectful care takes place, and that the ward team develop relationships with patients that involve active listening and decision making; no decision without me

Act as the patients advocate when appropriate, when informed decisions may lead to choices being made concerning treatment options.

Support daily board rounds using a coaching approach to develop nursing skills and enhance patient care.

Ensure that the delivery of care in the ward meets the individual needs of all patients, including single-sex accommodation, providing adequate nutrition, fundamental hygiene needs and considers inclusivity and personalisation.

Ensure that all patient care is assessed, planned, prioritised and high quality, both physical and psychological, delegating to support staff as required. Seek advice and support from other staff as required.

In accordance with individualised patient care, ensure clinical observations are taken, results recorded and any appropriate actions taken based on the results following local escalation plans.

Ensure the maintenance of professional contemporaneous records at all times by all nursing staff, documenting all actions and interventions in a clear, honest and timely manner in line with Trust policy.

Develop effective multi-professional teamwork, to ensure patients are reviewed as part of regular ward rounds, to include on-going care plans seven days a week, twenty- four hours a day.

Undertake regular quality rounds ensuring the plan of care remains appropriate for each patient and escalating any concerns in line with local standard operating procedures.

Ensure every medical ward round has a continuous nursing presence with proactive contribution to the plan of care, placing the patient at the centre of all decision- making.

Ensure ward nursing team monitor patient progress, work towards safe and timely discharges before lunch and ensure barriers to discharge are identified and acted on appropriately.

Deal with concerns raised by patients and visitors in a proactive manner, review patient feedback results and update the wards patient experience improvement plans, as needed.

Champion a safeguarding culture at ward level, which meets the needs of people with mental health problems and those with issues involving mental capacity

Ensure on arrival to the ward patients, relatives and other staff are received in a professional way and act in a courteous and polite manner at all times, thereby ensuring good relations between patients, public and staff.

To lead on infection control initiatives ensuring that all staff within the wards comply with Trust and NHS policies.

As a key member of a multi professional team, ensure appropriate referral and discussion with other members of the team to optimise patient care.

This role will specifically lead on criteria led discharge and other flow and pathway initiative across the care group and contribute to the work on these areas across the sites.

Education and Qualifications

Essential

NMC Registered Level 1 Completed degree in nursing Evidence of advanced patient assessment skills or ECAS Current experience of clinical nursing practice in a relevant large acute medical care setting. Current significant experience as a band 7 managing a team or department.
