They want to recruit someone with demonstrable experience of undertaking Occupational Therapy assessments and interventions for the generic Adult Social Care service within the City of London and to work as part of the Reablement service ensuring compliance with CQC requirements. The goal is to ensure the delivery of a responsive, high-quality service that offers value for money and meets the City's statutory requirements.

Please note: Whilst initially this is a temporary role, there is potential for the role to become permanent in the future. In this position, you'll be needed to work weekends and shifts during unsociable hours. Full details of how this will work will be revealed at the interview.
The Role:
  • Work alongside the social work team: Support a holistic, strengths-based approach to adults.
  • Manage hospital discharges: Assess risk and activities of daily living in conjunction with the Duty Senior Practitioner managing Adult Social Care Duty.
  • Assess and advise service users: Focus on all aspects of activities of daily living, always seeking to maximize independence and explore creative home-based alternatives to care home or hospital admissions.
The standard working hours for this position are 7.

The Candidate:

Qualifications: Social and health care (qualified) - Grade E Level 1 - Occupational Therapist.

Experience and Skills:
  • Ensure proper assessments are undertaken to meet service users' needs.
  • Recommend and order appropriate, cost-effective equipment.
  • Record data using MOSAIC and maintain detailed records of performance data, outcomes, and targets according to national and local indicators.
  • Develop good relationships with external partners/providers, clearly setting goals and outcomes for each distinct need and service.
  • Complete full OT assessments.
  • Implement rehabilitation plans with social work staff for reassessment.
  • Conduct assessments for Telecare, Taxi Card, and Freedom passes (independent mobility assessments).
  • Provide assessments and support for Disability Facilities Grants (DFG) and major adaptations, monitoring their development and management.
  • Manage and provide community paediatric occupational therapy, including assessments and technical support.
  • Manage a clinical caseload of generic Adults who meet OT eligibility criteria.
  • Undertake any other duties that may be reasonably requested and are appropriate.

  • Basic salary: Up to £25.23 per hour
