Job Overview

The post holder will act as a lead specialist within the ACT team, supporting and managing the ACT workforce within the service. Working closely with the ACT Lead nurse, the post holder will provide operational support, caseload supervision to the community sisters, and ensure strategic team objectives are achieved. They will also plan the specialist nursing service, education, training, and contribute to the development of services for individuals with acute conditions and care pathways across Swansea.

Main Duties of the Job

  • Provide operational support and caseload supervision to case managers
  • Facilitate team members' development to support individual and service objectives
  • Initiate research and innovations to enhance care quality
  • Maintain and disseminate knowledge within community networks
  • Support service delivery changes based on evidence-based standards and policies

Qualifications

Essential Criteria

  • RGN with a minimum of 5 years post-registration experience
  • Evidence of clinical/professional development
  • 1st Degree in a relevant subject
  • Master's level qualification

Desirable Criteria

  • Specialist area nursing qualification/experience
  • Community Nursing qualification

Experience

Essential Criteria

  • Managing patients/clients in a community setting
  • Experience in change and innovation in practice
  • Extensive experience in managing patients with chronic and acute diseases
  • Conflict management experience
  • Audit and evaluation experience

Desirable Criteria

  • Experience working with acutely unwell individuals
  • Evidence of project work

Knowledge and Skills

Essential Criteria

  • Ability to work in multidisciplinary teams
  • Understanding of the wider NHS agenda
  • Ability to support learning and education for patients and practitioners
  • Demonstrate understanding of clinical governance
  • Management and IT skills

Desirable Criteria

  • Presentation and negotiation skills
