The post-holder works under the direct supervision of DHCH Quality Manager - Medicines Optimisation Manager and undertakes tasks and duties delegated by them, following policy and procedures.


Responsible for supporting the Medicines Optimisation Manager in implementing effective medicines optimisation and eMAR management within the business, identifying areas for improvement, and initiating and managing change.


Responsibilities:

  • Monitoring of eMAR system(s) to ensure effective medication management across all services within DHCH.
  • Monitor eMAR systems to identify deficits in medication management.
  • Raising queries and providing support to services where deficits are identified.
  • Assist in the delivery of a robust, responsive, and safe system for prescribing repeat medications via eMAR system.
  • Assist in resolving prescription queries with Pharmacies/ Prescribers
  • Responsible for following the relevant protocol to; safely, appropriately, and efficiently:
  • Review prescription queries to identify why a medication was not prescribed/ received.
  • Use information systems and databases relevant to the role ( GP Connect)
  • Triage telephone enquiries services & wider multidisciplinary team in a prompt and professional manner, supporting good customer and resident care.
  • Liaise and build good working relationships with external pharmacies to help enable the prescription service.
  • Produce reports as and when required.
  • Contribute to achievement of the highest possible quality standards for medication management across DHCH.
  • Undertake additional tasks as required within your general role as Medicines Optimisation Assistant.
  • Must have a high standard of computer literacy.
  • Experience of monitoring electronic systems desirable
