University Hospitals Birmingham NHS Foundation Trust is pleased to invite applications to the post of Consultant in Emergency Medicine (EM) at Heartlands Hospital and Good Hope Hospital. Heartlands and Good Hope hospitals operate two large Emergency Departments which treat up to 800 patients per day between their departments. Successful applicants will have the opportunity to work across both Emergency Departments and although some cross-site working will be required, consultants will be based on a site where the majority of SPA activity will be focussed.

The successful candidate will join our team of enthusiastic, dedicated and hardworking consultants who share a goal to provide exceptional patient care and who strive to develop our service and improve our patient's experience. The current consultant body consists of colleagues with a range of specialist interests and roles including Director of Medical Education, Associate Deans for Education, Research Leads, Training Programme Directors, Trauma Leads, Ultrasound Leads, Medical Examiners as well as RCEM examiners to name but a few. Everyone's voice within our team is valued to harness innovation and improvement, we are always looking at ways in which we can develop our services and aspire to providing the best outcomes and address health inequalities to the diverse communities we serve.

As part of a large team, consultants are able to work flexibly to balance work and home life we also understand the importance of providing support to new consultants and place significant emphasis on wellbeing in a fast paced and demanding environment. The post holder will participate in the on-call rota. When on call, Consultants are only expected to support the emergency department of one hospital site.

The on-call arrangements amount to a 1:8 weekday on call rota, with a 1:8 weekend on call rota. There is an expectation that on-call duties are accompanied by an on-site presence until 10:30 pm. Consultants are expected to undertake a degree of cross-site working. Full time and part time hours are available. For those wishing to work part time, we recognise that the job plan will alter accordingly. Modifications to the job plan will be subject to discussion and mutual agreement prior to the successful candidates start date.

The existing Emergency Department Consultants provide high quality "shop floor" clinical presence and take pride in having positive feedback in trainee satisfaction for support and supervision. The team also strive towards providing high standards of education, training, research and audit with projects being recognised in the BMJ Awards.

The post holder will be expected to work as part of the Consultant team and contribute to the development of the department and Emergency Medicine as a speciality. The post holder will work closely with nursing, managerial and medical colleagues to provide high quality healthcare for patients.

1.SPECIFIC RESPONSIBILITIES OF THE POST:

Clinical

*The post holder will be expected to share responsibility for the delivery of high-quality care to the patients presenting to the Emergency Department

*To examine and treat patients in the Emergency Department

*To manage patients admitted to the Clinical Decision Unit (this was temporarily closed during Covid and post Covid years but with plans to be re-opened) and perform ward rounds and other supporting work as required

*To support the clinical decision making of all medical and nursing clinicians within the Department

*To provide on call support out of hours as agreed in the job plan. (A specific job plan will be agreed with the Clinical Service Lead on appointment and be subject to annual review)

*To attend the Department out of hours as per the agreed job plan.

*To remain contactable and available when on call

*To support the Department if required to do so in the event of a Mass Casualty incident

Service Development

Looking ahead and mapping the future path of Emergency care is considered to be highly important within our team. Alongside challenge comes opportunity. Our team recognises that there is a need to constantly develop patient care and rise to the challenges faced by the NHS as a whole. Our team have developed rapid assessment and treatment processes, a CDU pathway and a GP in ED service. The post holder will:

*Be responsible for clinical service development as agreed with the Clinical Service Lead

Teaching/Training (as agreed with Clinical Service Lead)

The Emergency Departments at Heartlands and Good Hope were innovators in developing extended nursing roles in the UK and have a well-developed contingent of Emergency Nurse Practitioners and Advanced Clinical Practitioners. These staff see, treat, refer and discharge patients across all clinical areas. The Emergency Departments also participate in the education and training of 5th year medical students from the University of Birmingham and Aston University, who are placed in the Departments and participate in the structured teaching programme. In addition to this we also facilitate training for physician Associates from University of Birmingham. The post holder will be required:

*To participate in the under- and post-graduate medical teaching programme

*To participate in the teaching programme for nursing staff

*To participate in the teaching programme for Emergency Nurse Practitioners

*To participate in the teaching programme for Advanced Clinical Practitioners

*To participate in the Senior Medical meetings, which take place on a weekly basis for Consultants and Middle Grade Medical staff

*To participate in the teaching programme for Specialist Registrars and Speciality Doctors

*To act as Educational Supervisors for Junior and Middle Grade Medical staff, including of appraisal and personal development planning.

Audit / Governance / Safety

The Team is regarded as being forward-thinking in its approach to governance. The cross-department publication of the "Risky Business" newsletter, which disseminates learning from significant events, has often been highlighted as demonstrating excellent practice in external reviews. This is supported by education in daily handover meetings. The department has developed apps to make clinical decision tools more accessible to doctors and on-line podcasts through a departmental website. The departmental website is currently under review and we are looking to energise this resource.

Research is a strong area within the department with the previously run HECTOR project being recognised in Central Research Network Awards and the 2018 BMJ Awards for its work in developing a Nation-wide approach to Silver Trauma. More recently the departments have contributed significantly to the CRASH 4 Trials. Many opportunities exist to continue along this line of research or develop new ideas and new modes of practice. The post holder will be required:

*To promote evidence based practice

*To develop, review and update Departmental guidelines and SOPs as required

*To change own and other's clinical practice in response to new guidance and / or

developments in clinical practice

*To undertake and supervise relevant clinical audits and /or quality improvement projects

*To actively participate in all aspects of risk management in the Departments

*To remain up to date and engaged in CPD as per CEM guidelines

*To respond appropriately and speedily to complaints and critical incidents,

including conducting investigations and developing action plans to preventrecurrence.

Clinical Governance

*To work in accordance with the Trust's Standard Operating Procedures andpolicies.

*To attend accredited conferences and meetings to update own clinical practice,

teaching and management skills, in line with CME requirements.

*To participate in the annual job planning process

*To participate in the Trust's annual appraisal process

*To ensure compliance with Trust policies and procedures

Management

*The post holder is expected to take responsibility for managing the Departments. This will include day-to-day management such as review of abnormal X-rays and blood results with patient follow up to ensure they are acted upon, responding to complaints and other general administration/correspondence.

*The post holder will be encouraged to take on specific elements of service development within the Departments. This will be mutually agreed following discussion with the Clinical Service Lead and incorporated into the post holder's job plan. In addition, the post holder will be supported and encouraged by the Directorate to develop their own areas of interest, which contribute to the wider development of Emergency Medicine as a speciality.

*With mutual agreement of CSL/Clinical Leads, the post holder is expected to deputise and represent the Departments as required, both within the Trust and externally.

We are a team inside and outside of the department and many colleagues attend professional, social and charitable events amongst other things to help strengthen relationships. The post holder will be required:

*To work within the Emergency Department team and engage with the

Multi-disciplinary team to facilitate clinical decision making.

*To engage and collaborate with colleagues from other Directorates.

*To develop maintain productive working relationships with colleagues across the wider Health Economy.

*To show initiative, enthusiasm and flexibility.

Accountability

*The post holder will be immediately accountable to the Clinical Service Lead. The post holder will be expected to undertake yearly appraisal as per Royal College of Emergency Medicine / GMC guidelines and to respond to any identified development needs. Annual appraisal in line with revalidation is expected to occur as a GMC requirement.

*Entry on the GMC specialist register for Emergency Medicine or within 6 months of CCT
