An exciting opportunity has arisen for a 12-month fixed term role for an experienced and enthusiastic pharmacist to lead medicines optimisation as part of our multi-morbidity model of care (MMMoC) project to optimise medicines and support integration of services and pathways for patients with chronic kidney disease and cardiometabolic disorders across South East London.

The project is a collaboration between primary and secondary care colleagues to create a person-centred, holistic integrated model of care for people with multiple long-term conditions to improve patient experience, change ways of working and increase efficiencies.

This post is one of two roles hosted by Guy's and St Thomas' NHS Foundation Trust and King's College Hospital NHS Foundation Trust however will work across multiple sites including Lewisham and Greenwich NHS Trust and primary care.

The successful candidate will:

  • have significant experience as a specialist clinical practitioner and act as a role model in chronic kidney disease and/or cardiovascular disease and/or diabetes
  • be able to demonstrate a commitment to patient care
  • be able to demonstrate the ability to influence at consultant and senior leadership level
  • be able to work innovatively and demonstrate experience of service development, and improvement across a system wide multi-disciplinary environment
  • have excellent leadership, facilitation and communication skills
Main duties of the job The main duties of the job will be to optimise medicines for patients with chronic kidney disease and cardiometabolic disorders across South East London structured around the key areas outlined below:

Leadership

Candidates must have successfully completed the Postgraduate Diploma in Clinical Pharmacy Practice (or equivalent) and be a registered Independent Prescriber.   • This newly created post will focus on optimising medicines and improving outcomes for patients with chronic kidney disease and cardiometabolic disorders across South East London.
  • This is an excellent opportunity for an aspiring leader to take on service development project, work collaboratively across the MDT, oversee the development of appropriate pharmacy governance pathways for this speciality and undertake and support research in this clinical area.
  • All of these opportunities will cumulate in allowing us to provide the best possible care to the patients who use our service.
  • The role will be responsible for the line management of a small team and have the benefit of being mentored and supported by the Consultant Pharmacist - Diabetes and Principal Transplant, Renal and Urology Pharmacist
Person Specification Professional / Statutory Registration Essential
  • Registration with General Pharmaceutical Council
Qualifications / Education Essential
  • MPharm degree (or recognised equivalent)
  • Postgraduate Clinical Diploma in Pharmacy Practice or equivalent training, experience.
  • Non-medical prescribing qualification and registered with the appropriate healthcare regulator as a non-medical prescriber.
Desirable
  • Project Management Qualification (or comparable experience)
  • Member of relevant professional groups (eg UKRPG, RPS, UKCPA)
Skills and Abilities Essential
  • Plan, implement and evaluate programmes that impact across or within departments, services and organisational boundaries
  • Effectively challenge current practice using negotiating, motivational and influencing skills
  • Consistently provide professional accountability to patients
  • Ability to work weekends / bank holidays and support out of hours rosters, when required and as appropriate

Knowledge and Experience Essential
  • Understanding and demonstrable knowledge of Equality, Diversity and Inclusion.
  • Experience of having led a clinical pharmacy service for patients living with chronic kidney disease and/or cardiovascular disease and/or diabetes or experience of running clinics or providing expert clinical advice for people living with chronic kidney disease and/or cardiovascular disease and/or diabetes.
  • Experience of managing and making appropriate referrals.
  • Experience of partnership work across the wider health economy and outside of organisational boundaries.
  • Expert advanced ability to ensure safe, clinically effective and cost-efficient use of drugs in chronic kidney disease and/or cardiovascular disease and/or diabetes.
  • Hold expert advanced clinical knowledge in medicinal therapy in chronic kidney disease and/or cardiovascular disease and/or diabetes.
  • Able to identify new prescribing practices that may impact on budget (horizon scanning).
  • To be able to demonstrate knowledge and understanding of commissioning
  • Evidence to effectively challenge current practice using negotiating, motivational and influencing skills.
  • Able to work collaboratively with senior pharmacy, medical staff, the multidisciplinary team, management and commissioners, outside and within organisational boundaries.
  • Evidence of undertaking own research.
  • Have evidence of the ability to educate, train and evaluate pharmacy staff and other health care professionals in medicinal therapy in chronic kidney disease and/or cardiovascular disease and/or diabetes
  • Hold advanced expert knowledge in Clinical Governance Agenda with specific focus on medicinal therapy.
  • Broad experience of clinical pharmacy services within chronic kidney disease and/or cardiovascular disease and/or diabetes.
  • Enhance the quality of patient care.
