This is an exciting time to join the team at Pathfinder Stockport. We provide a non-discriminatory and user friendly service, offering recovery focussed interventions to reduce substance misuse related harm for individuals, their families, and the local community. We work in partnership with wider health and wellbeing services to meet the diverse needs of Stockport residents, and to deliver a high standard of care.

All new starters to Pennine Care NHS Foundation Trust who are appointed to a post which requires a Disclosure Barring Certificate check (DBS) will be recharged for the full cost of the check. Substantive staff will have the amount deducted from their first months' salary.

Main duties of the job

Role Summary:

A substance misuse worker, providing recovery focussed interventions to people whose alcohol and drug misuse has resulted in a range of offending behaviour.

Key Responsibilities of the Post:

To actively manage the recovery journeys of a case load of drug and alcohol clients with complex needs To actively manage a caseload of clients receiving OST To support both drug and alcohol clients through community and in-patient detoxification programs. To formulate, implement and review care plans. To provide reports in relation to Child and Adult Safeguarding and attend Child In Need and other safeguarding meetings

Person Specification

Education and Qualification

Essential

Evidence of specialist training (eg. NVQ Level 3 in Community Health or Drug and Alcohol Use Practitioner course. Evidence of, or willingness to work towards, 10 DANOS competencies. Short courses and experience to graduate level

Desirable

Relevant professional qualification RGN / RMN / DipSW Relevant training in specialist area. Training in sexual health / blood borne viruses.

Experience

Essential

Significant experience of working with a variety of alcohol issues and interventions. Significant experience of delivering evidenced based interventions for a variety of other substances Significant experience of assessing clients' health and social care needs in a drug or alcohol setting. Significant experience of care planning , risk assessment and risk management as it relates to the substance misuse sector Significant experience of providing advice and support on health and / or social issues. Significant experience of multi-disciplinary and multi-agency working. Significant experience of delivering effective interventions to individuals and groups who have drug and / or alcohol problems Significant experience of providing support to vulnerable / socially excluded individuals / groups Significant experience of working as part of a team in order to achieve a shared vision / goal

Desirable

Experience of community engagement and service user and carer involvement Experience of working in a primary care setting Experience of delivering Health and wellbeing interventions Experience of prescription / medicines management as it relates to the substance misuse sector. Knowledge

Essential

Knowledge of the physical, psychological and health effects of drug and alcohol use and appropriate interventions. Knowledge of therapeutic techniques ( such as motivational enhancement) Knowledge of Brief interventions and extended Brief interventions Understanding of the recovery and abstinence agenda and ability to apply this to practice. Knowledge of treatment options and resources available to support problem alcohol users. Knowledge of issues relating to Drug and Alcohol and understanding of the principles of harm minimisation Knowledge of the importance of mutual aid and peer support networks Understanding of local communities and their diverse health and well-being needs An awareness of policies and procedures in relation to confidentiality, information sharing, and child protection

Desirable

Knowledge of therapeutic processes including wither social networks ( systemic approaches) Knowledge of Health and social Care priorities as they relate to Primary Care. Knowledge of national substance misuse strategy ( Models of Care, QUADS, DANOS)

Skills and Abilities

Essential

Ability to assess, care plan, risk assess and risk manage as it relates to the substance misuse sector Case management skills. Ability to deliver a range of evidence based interventions for a variety of alcohol related issues. Ability to deliver Brief interventions and extended Brief interventions Ability to deliver evidenced based interventions for a variety of other substances Ability to work with clients on a one to one and via group work Ability to acknowledge diversity and promote anti-discriminatory practice / equal opportunities Able to demonstrate full understanding of confidentiality issues and the ability to interpret policy into practice. Ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging behaviour Ability to communicate drug and alcohol specific knowledge and expertise understanding of recovery capital, asset based frameworks, building social networks and importance of mutual aid. IT skills in relation to data input and statistical monitoring.
