We are looking to recruit a Mental Health Practitioner ? Care Coordinator to work in the Stockport Early Intervention Team.

The Early Intervention a multi disciplinary team with staff from a range of professions including, mental health nurses, social workers, CBT therapists and support workers. The team has a dedicated Consultant Psychiatrist, Advanced Practitioner, a Psychology lead and full admin support.

You would be responsible for a defined caseload of individuals experiencing first episode psychosis as a care coordinator / mental health keyworker within the team, as such you would work with individuals to meet their identified needs using outcome measures, a recovery approach and facilitate discharge from secondary mental health services, or referral on as appropriate. Additionally the post holder would contribute to duty and clinic work and participate in team development issues.
