Job summary

Working with young people experiencing poor mental health across Bristol, who are in secondary mental health services and helping them and their mental health professionals in mapping and transitioning into a pathway to manage their mental health beyond their 18th birthday.

The post holder will ensure a cohesive, smooth transition for young adults from children and adolescent mental health services (CAMHS) to adult mental health services (AMHS) that is evidence as being significant in terms of their longer term recovery.

They will also provide expert advice and support to all relevant stakeholders with regards to young adults who are receiving support from secondary mental health services who are approaching their 18th birthday.

NICE guideline detail good practice in respect to the transition process and will help inform future practice.

Main Duties:To work across Bristol, supporting both CAMHS and adult recovery services in establishing pathways plans for young people who experience difficulties with their mental provide comprehensive and specialist advice around the development of a care pathway for a young person experiencing emotional dysregulation who is transitioning into work with professionals and carers for the young person to ensure transition is plan, prioritise and work alongside the young person to ensure the transition process is clearly understood and managed a person if identified as requiring support from Recovery services, to ensure that a care coordinator is identified within AMHS and that joint working is undertaken during the period of ensure that any other multi agency input is in place to work with and on behalf of young people in a way that promotes and develops their efforts, resources and quality of life. To deliver services in a manner that is appropriate, relevant and respectful to all community members, recognising diversity of ethnicity, culture, belief, privilege and liaise and work in partnership with staff from other statutory services, voluntary agencies and community groups that work with facilitate and improve pathways from third sector/statutory services to relevant adult and to provide advice and to assist other professionals working with young people with interventions which draw on own specialist professional provide advice and support to carers/ relatives of young participate in clinical meetings CPA and client reviews as link in as required with other services within AWP (Early Intervention, BASS, STEPS, ADHD service) when required as means of supporting as smooth a discharge as possible (noted that these services mainly manage their own transitions).To support the full implementation of the Care Programme ensure that legal and Trust requirements affecting service user and staff are adhered to, and ensure a safe working environment for service users and staff. To cooperate with statutory agencies with the responsibility to protect children and ensure that issues of risk and safety of young people are assessed and any necessary action is maintain a high degree of professionalism at all times when working with highly distressed line with Trust policy, to receive regular clinical and management maintain accurate records relating to clinical and professional activity and to monitor clinical workload using agreed communicate complex and sensitive information to patients, carers and professionals,including information about diagnosis and prognosis, to find ways of enabling young people and their families to take this information on board, and to provide empathy and reassurance. Most challenging part of this role:To be part of developing multi-disciplinary and partnership working between CAMHS, AMHS and psychological therapies. Promoting links between agencies that work with young people and statutoryservices. To develop care pathways for clients who have complex presentations.

Essential

RMN, Occupational Therapist or Social Worker qualified to degree level or can demonstrate equivalent work experience Holds a current professional registration with their governing body Relevant experience working alongside community care coordinators Relevant experience working with individuals with emotional and attachment issues. A working knowledge of psychosocial interventions. Sound understanding of core components of interventions to support mindfulness and emotional resilience Experience of care coordination Understanding of current policy and guidance relating to CAMHS/AMHS transition. Understanding of the needs of particular groups, young people, residents of inner city, rural, black and minority ethnic groups and skills in engaging with them. Experience of working within both CAMHS and AMHS settings or extensive knowledge of both services Previous experience of working with young people Previous experience of working with people with Emotional Dysregulation Experience of working with families Experience & understanding of clinical Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources.
