As a pivotal member of our council, you will play a crucial role in facilitating the transition process for adults, ensuring their well-being and providing vital support during this significant life phase.


Responsibilities:

Manage a caseload involving assessments, support plans, and reviews with adults.

Complete Court of Protection documents, best interests assessments, and safeguarding investigations.

Assess risk factors and explore suitable placements (Supported Living, residential, and housing) in collaboration with health professionals.

Requirements:

Proven experience in handling all aspects of the tasks outlined.

Ability to support the team and seamlessly integrate into existing workflows.

Proficiency in IT experience with Liquid Logic preferred, but training can be provided if necessary.

Availability for immediate interview and start date.

A recognized Social Work qualification and registration with Social Work England.
