Care Collaborative Primary Care Network (PCN) has an exciting opportunity to expand their Primary Care Workforce by employing a Children and Young Persons Care Coordinator and implementing a CYPSP (Children and Young People Social Prescribing) service for our patients.

We would like to build on our existing PCN team as we continue to recognise the value the additional roles bring to our practices and patients.

Our aim is to provide exemplary patient care; find innovative solutions in general practice to deliver the best care we can for our patients.


Within Care Collaborative PCN there is an identified need for a Children and Young Persons Care Coordinator to work with a Children and Young Persons Social Prescriber and Health and Wellbeing Coach, to support young people's health and wellbeing and helping them to navigate through the different support networks available to them and to coordinate the support provided by the new service.

Our Primary Care Network comprises of 3 practices covering Redhill, Reigate and Merstham. The practices have a combined population of 49,000 patients.

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.

You will be joining an enthusiastic team of clinicians and administrators. The role will contribute to improving the quality of care of our patients across the network of practices.

Main duties of the job


This role is to support the smooth co-ordination of care across the Care Collaborative Children and Young Persons Mental Health service.

This is a new service that will require support to model and shape the service.


The Children and Young Persons Care Coordinator will support the Children and Young Persons Social Prescriber and Health and Wellbeing Coach in identifying and managing a caseload of patients, referring patients to health and social professionals as needed and providing and organising PCN Multi-Disciplinary Team Meeting on a regular basis.

You will work very much in partnership with clinical and non-clinical colleagues to ensure you deliver the best possible outcomes for our patients.

Job description

Primary duties and areas of responsibility

  • To work across a Primary Care Network (PCN) supporting a wide range of patients aged 019 (up to 24 years with learning difficulties) and coordinate the navigation through the Primary Care Network led Children and Young Persons Mental Health Service.
  • Coordinate the referrals into the Children and Young Persons Mental Health service and onward referrals to other agencies.
  • To utilise soft and hard intelligence within the PCN to identify patients who will benefit from a proactive approach by the PCN, the wider community and multidisciplinary teams.
  • To identify patients to refer to the Children and Young Persons Mental Health team, and if required, discuss at PCN level MDTs where a multiple professional group attend to discuss the most complex patients.
  • Coordinate MDT meetings with the PCNs and supporting the running of MDTs independently, ensuring that relevant professionals are in attendance.
  • Uploading and maintaining patient care plans to EMIS and other relevant systems as necessary.
  • Maintaining action logs to audit outcomes and being able to give updates to professionals from multiple providers across the PCN, including the patients, carers and next of kin if appropriate.
  • Keeping up to date records in all GP clinical systems across the PCN, including data that can be reported on and shared with the PCN for outcomes, on a monthly basis.
  • Maintaining up to date and well organised records in the Children and Young Persons Mental Health Teams channels.
  • Identifying patients with and without care plans, to ensure they are up to date.
  • Liaising with the Children and Young Persons Social Prescriber and/or Health and Wellbeing Coach for patients that are identified as needing wellbeing and/or coaching support.
  • Ensuring all patients within the Children and Young Persons Mental Health Service have booked appointments and follow ups if necessary.
  • Ensuring Templates are completed by professionals in order to provide accurate data, by encouraging there use and auditing regularly to ensure adherence.
