Senior End of Life Care Facilitator

This post is a team leader role based within the specialist palliative care team.

The team leader will be responsible for the urgent response team, the end of life care facilitators in the co-ordination Hub and the out of hours DNs.

The applicant will need extensive experience with end of life care and community working.

The team leader will supervise and develop a team of band 6 and band 5 registered nurses.

The team leader will offer clinical oversight and leadership, this will include managing staffing issues such as recruitment, staff well being, retention, sickness and training and development.
