The Older Persons Mental Health Service (OPMHS) provides a comprehensive, specialist mental health assessment and treatment service to older people with mental health problems in the community.

As a Community Mental Health Professional you will be responsible for:

Taking a lead role in the completion of holistic, first line assessment of new referrals to the OPMHS Formulating, evaluating and reviewing of individualised, therapeutic plans of care Demonstrating a high level of competence in the assessment and management of clinical/environmental risk Delegating and referring work to other professionals on a daily basis within the multidisciplinary team in order to meet identified patient/carer needs Utilising evidence-based interventions in your practice in accordance with meeting identified social, physical and mental health needs of patients and their carers Holding a BSc or equivalent in mental health nursing/social work or occupational therapy, and having comprehensive experience in mental health care is essential.

The ability to demonstrate a commitment to working in an integrated multidisciplinary team environment is also required.
