**About the role and our team**

We are looking for a skilled, knowledgeable and enthusiastic health professional to join our Older Person Community Team. The Older Persons Mental Health Service (OPMHS) provides a comprehensive, specialist mental health assessment and treatment service to older people with mental health problems in the community.

As a Community Mental Health Professional you will be responsible for:

- ** Taking** a lead role in the completion of holistic, first line assessment of new referrals to the OPMHS
- ** Formulating**, evaluating and reviewing of individualised, therapeutic plans of care
- ** Demonstrating** a high level of competence in the assessment and management of clinical/environmental risk
- **Delegating **and referring work to other professionals on a daily basis within the multidisciplinary team in order to meet identified patient/carer needs
- ** Utilising **evidence-based interventions in your practice in accordance with meeting identified social, physical and mental health needs of patients and their carers

Holding a BSc or equivalent in mental health nursing/social work or occupational therapy, and having comprehensive experience in mental health care is essential. The ability to demonstrate a commitment to working in an integrated multidisciplinary team environment is also required.

Housing Assistance is also available where the Department will pay the difference between your current mortgage/rent bill, and that of your new accommodation on the Isle of Man, up to a maximum of £250 per month.
