keyworker**Aim of Post:To provide high quality generic support including advice and information, assessment and referral, key-working, motivational interviewing, brief interventions and care-planned treatment to individuals and groups in a community setting using shared accommodation.
Requirements of the Post
- To provide person centred support regardless of the service users' issues
- To work closely with referring agencies to provide support and accommodation for homeless people, ex offenders and rough sleepers.
- To support service users in maximising the benefits of the service and to introduce them to other professional and charitable services which will support them with drug and alcohol issues, debt counselling, primary and specialist health care, personal and housing benefits.
- Identify the individual's needs through the assessment and care planning process agreed with the client and the offender manager or advocate as the case maybe.
- To prepare daily reports in relation to contact, physically or by telephone. To report in writing any issues or activities the individual has taken part in or will take part in. This is to include any anti social behaviour.
- To ensure a safe and effective service by developing a relationship with the service user and by regularly assessing progress through a care plan review.
- To work as part of a team to provide accessible and welcoming services for all service users.
- To submit reports and data in an accurate and timely manner.
- To maintain case files and client records in an accurate and timely manner.
- To identify service users needs through accurate assessments and to make and support them into the appropriate service through key working and other interventions.
- To participate in three way reviews with offender manager, drug and alcohol and service users.
- To work with external agencies to ensure successful onward referrals.
- To provide harm reduction, advice and information.
- To directly provide support in addressing their substance use through individual and group interventions, provided by statutory services.
- To attend regular supervision and team meetings as required.
- Attend training as required.
- To work closely with referring agencies to provide the voluntary day programme element of the clients treatment package.
- To undertake any reasonable management instruction as required.

- Driving licence and access to own car
