Contract:
Until 30th June 2023 with potential to extend for a further 2 years

Responsible to:
POPS' Operations Manager

PURPOSE OF THE POST


The post holder will be based at POPS' Welfare hub in Manchester and will operate on a case work basis, to carryout initial assessment, provide 1-1 support and deliver a range of small group sessions to address and resolve identified welfare/wellbeing issues.

The role will peripatetic, requiring travel to various community and Probation locations across Manchester

TASKS

  • Manage case load of mandated referrals from Probation Service
  • Carry out needs assessments with all individuals referred to the Welfare Programme Outcome Star Assessment Tool.
  • Produce and agree an individual action plans within the timescales set by The Probation Service national standards
  • Deliver advice, support and interventions to people on Probation.
  • Make supported referrals for individuals to access to specialist provision where required, i.e. Debt management services and track progress
  • Provide timely and accurate information to Probation Practitioners
  • Provide culturally appropriate support for Men and Women from diverse backgrounds
  • To work with each person on Probation to achieve agreed goals, milestones and outcomes by providing a diverse and bespoke range of 11 support and small group work interventions
  • Coordinate service user referrals to partner agencies for specialist interventions, support, or activities (i.e Mental Health Services.
  • Maintain up to date case records using Probation service and POPS' In House electronic CRM systems.
  • Work on flexible basis, some travel across locations in Manchester will be required
  • Undertake any other duties as requested by POPS' line manager.
