We are currently seeking a committed Practice Nurse specialising in diabetes care managment to join our team at Tonbridge Medical Group (TMG).
Job Summary
To be responsible for providing comprehensive diabeticmanagement and support for the whole practice population. With a focus oneducating and empowering individuals to better manage their condition andoffering high quality care to our patients. Additionally to be responsible forthe implementation of processes for the effective management of patients withlong-term conditions using evidence-based practice including care for elderlyand housebound patients.
Working as a member of the practice multidisciplinary team, thepost holder will ensure nursing services are delivered effectively to theentitled patient population.
Main duties of the job
Duties include but not limited to:
Summary of core responsibilities:
Deliver timely and professional diabetes care
Identify and assess patient's diabetes care needs
Provide specialist advice in diabetes management
Develop and implement health promotion programs
Implement and evaluate individual treatment plans for chronic disease patients
Perform cervical smears
Manage and support patients at risk of long-term conditions
Provide routine nursing care in accordance with clinical evidence
Manage acute and chronic conditions
Suggest medications for patients within scope of practice
Co-ordinate travel medicine services
Request and process pathology services
Maintain accurate clinical records
Oversee chronic disease clinics and referrals
Ensure compliance with practice policies
Prioritise health issues and intervene appropriately
Support team in clinical emergencies
Recognise and refer patients with mental health needs
Implement vaccination programs
Support patients with prescribed medications
Liaise with external services for patient support
Delegate clinical responsibilities safely
Support team with safeguarding matters
Refer patients with substance abuse/addictive behavior
Deliver health promotion initiatives
About us
Tonbridge Medical Group has a list of 17,600 patients. We are a dedicated & passionate team working in a state-of-the-art facility in the centre of TMG, we prioritise forward-thinking healthcare delivery and strive to provide the highest level of care to our patient community.
As a training practice, we have 7 partners, 7 salaried GPs, 4 GP registrars, a physician associate, clinical pharmacists, nurses, healthcare assistants, a phlebotomist, and exceptional admin and care navigation teams.
In addition, TMG is part of the Tonbridge Primary Care Network (PCN) and is supported by a fantastic PCN team.
Job descriptionJob responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. To ensure that the Practice's diabetes care is delivered in a timely, professional manner whilst maintaining equity of access to a coordinated and seamless service.
b. To identify patient's diabetes care needs using a holistic approach and to assess and offer specialist advice in in diabetes management patients.
c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
d. To develop, implement and embed health promotion and wellbeing programmes
e. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
f. Performing cervical smears
g. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
h. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
i. As required, to manage acute and chronic conditions
j. To suggest medication for patients in accordance with PGDs and own scope of practice
k. To review patient medications to enhance compliance
l. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
m. To co-ordinate the provision of travel medicine services
n. To request pathology services and process pathology results as required
o. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
p. To maintain accurate clinical records in conjunction with extant legislation
q. To ensure read codes are used effectively by all members of the nursing team
r. To ensure the appropriate maintenance and use of chronic disease registers
s. To ensure compliance with the practice chaperone policy
t. To assist GPs with minor surgery when an HCA is not available
u. To prioritise health issues and intervene appropriately
v. To support the team in dealing with clinical emergencies
w. To recognise, assess and refer patients presenting with mental health needs
x. To implement vaccination programmes for adults and children
y. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
z. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
aa. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
bb. To support the clinical team with all safeguarding matters, in accordance with local and national policies
cc. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
dd. To deliver opportunistic health promotion where appropriate
Secondary responsibilities
In addition to the primary responsibilities, the Practice Nurse may be requested to:
a. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle
b. Participate in local initiatives to enhance service delivery and patient care
c. Support and participate in shared learning within the practice in order to improve patient care
d. Participate in the review of significant and near-miss events applying a structured approach, , root cause analysis (RCA)
