Our Integrated Community Teams collaborate with primary care and a range of community services to manage the care of housebound and high-risk patients. In this role, you will focus on supporting admission prevention, early discharge support, promoting independence, and effectively managing patients with chronic conditions.

This position requires flexible working hours as our service operates from 8am-8pm, 7 days a week. However, we are open to discussing and accommodating a schedule that suits you best.

If you have never worked in a community setting before, worry not We are willing to offer an extended induction period and preceptorship to all newly qualified staff.

All applicants must possess a valid driving license, professional registration, and access to a vehicle for travel (with mileage allowance provided).

Main Duties:

Working within professional guidelines and accountability, you will manage a defined community caseload for a group of GP Practices as part of the Integrated Care Team. Your responsibilities will include coordinating referrals based on clinical needs, conducting holistic assessments, and maintaining care plans in line with industry standards. Ensuring high standards of patient safety and quality outcomes will be paramount in your role.

You will work autonomously with a strong sense of professional accountability, with a key objective of preventing unnecessary hospital admissions and facilitating timely discharges into the community. Additionally, you will be involved in re-enablement, rehabilitation, and health promotion initiatives aimed at enhancing patient outcomes and reducing inequalities.
