The NHS Long Term Plan requests that all systems have access to an improved emergency response for mental health in conjunction with the existing emergency services.

It is accepted that a large proportion of conveyance to or attendance at Emergency Departments could be avoided in the community with a more timely response by existing mental health crisis services.

We also believe that a more local response to individual crisis would see service users supported much closer to their homes in more appropriate pathways and alternatives to existing crisis locations.

This will be a service that can respond to community emergencies within 1 hour rather than the current 4 hour response.

We will seek to respond to all EMAS/IRH and Professionals requests for an emergency response by mental health services. This will include crews at incidents and any agreed deployments from the EMAS control room. We would also look to support with appropriate mental health transportation and mental health act related activity.


This service will model a Trauma Informed based approach and a training package will be delivered to staff as part of their specialised induction along with other bespoke and service tailored sessions.


This roles requires high level interpersonal skills and experience of working with individuals who are in crisis as well as a sound knowledge of existing Community resources.

Safety planning and practical support to service users and the support to their Carers is critical within this role.


To liaise and communicate effectively with statutory and non-statutory mental health agencies regarding identification of individuals with mental health needs who are admitted to the general hospitals.

To maintain protocols for effective communication between both local mental health services, 3rd sector partners and other interested organisations.


To support colleagues in specialised risk and mental health assessments of individuals engaged with the crisis care pathway to inform recommendations for their care/treatment.

To promote and develop effective working relationships with key agencies.

To be able to work effectively in a range of community based settings.

To follow a Trauma Informed approach and to be a Champion of this approach within NHFT.


To support colleagues as a focal point for the identification and planning of aftercare for individuals within the Crisis Care Pathway.

Improve the experience of patients with mental health illnesses who require support from the crisis care pathway.

Provide perspective to multi-disciplinary team members regarding assessment, intervention, management and safety planning.

Provide a service to the Crisis Care Pathway countywide.

Contribute to support, training and skill development for individuals and groups in mental health awareness.


To assist the Band 7 Clinical Team Lead and the rest of their Team with the day-to-day management of the Crisis Response Unit.


To promote and develop effective working relationships with key groups with the general acute hospitals and associated community based teams, EMAS, Police and 3rd sector colleagues.

To work as a key member of the Crisis Response Unit.

Offering a range of interventions for individuals with a wide range of mental health difficulties, in line with the service operational policy.


To offer education, opportunistically to individuals and Carers in order to improve their knowledge and skills of mental health, and reduce the stigma of mental health.


There will be an expectation that out of hours support will be provided within this role, and that the post holder will be able to provide cover for the service Countywide, working on a shift pattern across a 24/7 basis.


Supporting colleagues with mental health assessments of individuals within the crisis care pathway, addressing the holistic aspects of their health needs.


To participate in a comprehensive biopsychosocial assessment process and be part of developments that move towards a need based response system that seeks to build on the Hope, Control and Opportunity of service users and Carers.


To support with the formulation and implementation of appropriate care plans/packages of care for the individual prior to their discharge to ensure that their mental health care is seamless throughout the care pathway.


To promote co-production with the Service User and Carers in all processes and interventions throughout the services within the pathway.


To ensure at all times that service users are involved in the Biopsychosocial assessment process, to ensure that all care pathways are individualised in their form.


To work in collaboration with acute hospitals, external mental health agencies, and Police in order to address mental health needs of the individual.

To work collaboratively with colleagues throughout the crisis care pathway and the wider Specialty Services.

To support others in the pathway.
