We are looking for a Band 6 Care Coordinator to be part of the Early Intervention team. Check out the details below:

  • Working hours: Monday-Friday, 9am-5pm
  • Contract length: 3-month initial contract with potential extension
  • Location: Hounslow
  • Rate of pay: £27p/h Umbrella

Key responsibilities include:

  • Providing early detection and intervention for individuals aged 14-65 facing a first psychosis episode.
  • Implementing evidence-based treatments to support individuals and families affected by psychosis.
  • Managing patient caseloads and ensuring comprehensive care coordination.
  • Conducting initial assessments and duty tasks efficiently and professionally.

About the team: The service focuses on early detection and intervention for individuals experiencing a first episode of psychosis. Our goal is to provide effective treatments to minimize the impact on individuals and their families.

Requirements:

  • Must be a Registered Mental Health Nurse with a valid NMC Pin
  • Prior experience in a community setting
  • Essential experience in EIP (Early Intervention in Psychosis)
