Responsibilities:

  • Promote independence and reduce risks for Adults with disabilities in Doncaster.
  • Work with the social model of disability and use evidence based practice for assessments.
  • Respect individual values, cultures, and diversity to identify needs.
  • Offer information and advice on self-directed support.
  • Provide expert knowledge on manual handling and support colleagues.


Duties and Responsibilities:

  • Conduct assessments of occupational performance with Clients with various conditions.
  • Develop individual action plans based on physiological, psychological, and environmental factors.
  • Involve the person and their carer(s) in decision-making.
  • Manage complex cases and reduce risks for individuals in their homes.
  • Provide clinical advice and intervention on manual handling.
  • Take part in triaging referrals and promote assisted technology for independence.
  • Undertake assessments for Blue Badge applications.


Requirements:

  • Occupational Therapy qualification (DipCOT/Degree OT).
  • HCPC registration.
  • Previous experience in Social or Health Care setting.
  • Experience in delivering OT assessment for Clients with complex needs.
  • Knowledge of Health and Social Services for disabled individuals.
  • Inter-personal and communication skills.
  • Experience with technology systems and Enhanced DBS check.
