Actively integrates theory and practice. Act as a 'key worker' for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients. Assesses people's information needs and provides information, to determine their own future. Enables patients access, identify, appraise and interpret complex technical information to help them to determine their own future. Provides clinical expertise, specialist advice and support across service boundaries. Leads and initiates the nursing contribution to service development, ensuring the pursuit of excellence in care. Defines and initiates a clear care pathway, refers to other professionals and signposts to other agencies as appropriate. Promotes the service and clinical excellence by contributing to publishing and presenting innovations, audit and research locally, regionally and nationally. Influences and develops policy and clinical guidelines internally and externally. Promotes self-management, risk stratifying patients into who can self-manage following education, those who will need guided support to self-manage and those who will require ongoing, face to face support, in order to support people to live with and beyond necessary treatments. Designs, directs, implements and evaluates educational offerings based on knowledge, skills and learning levels of nursing staff and allied health professionals. Develops and utilizes appropriate evaluation systems to determine the effectiveness of educational activities. Consults with individuals within the Nursing Institute and Ambulatory Nursing, CCL and the community regarding professional nursing practice and roles. Assists in the development of department specific orientation and competencies. Assists departments in orientation of staff. Monitors and evaluates nurse performance during orientation in conjunction with nurse preceptor Develops and maintains a cataloguing system for tracking and retrieval of all available teaching-learning resources. In conjunction with the senior team develop and monitor clinical and non-clinical performance standards being responsive to changing Provides consultation of a clinical and educational nature to department staff Applies current theory and research findings to clinical practice Administers unit based in-services Monitors and tracks nurse achievement of educational requirements Serves as a resource for the application of nursing policies and procedures Resources staff related to medical equipment and nursing informatics Performs other duties as assigned

Leadership & Collegiality

Act as a role model, demonstrating high standards of care and providing clinical leadership to others. Acts as an active and collaborative member of the MDT, ensuring robust clinical decision making and strong supportive governance structures are in place. Promotes a culture that supports and enables self-management. Provides specialist education and training to other professionals involved in patient care. Leads the development, management and promotion of services, ensuring the delivery of high quality, cost effective care. Embeds patient and public involvement within the sphere of practice. Promotes the philosophy of person centre care, co-creating care with people affected by cardiovascular disease. Collates and records quantitative and qualitative data to provide evidence of productivity, outcomes and quality, through audit and research, and provides the Department and Company with an Annual Report demonstrating the evidence. Initiates regular Service Reviews in order to drive service improvement. Leads on the implementation and monitoring of adherence to relevant clinical guidelines. Takes professional responsibility for ensuring effective communication between all service providers across the care pathway. Actively participates in multi-professional meetings, senior management team meetings acting as patient advocate and representing nursing and service views. Makes sure that patients are aware that they are interacting with know the full range of resources and services available within and beyond CCL, including appropriate charities. Actively engages with Chair to contribute expertise Experience and supports CCL Services priorities. Acts as a role model for excellent advanced communication skills and expertise. Demonstrates skills in conflict resolution and negotiation skills when dealing with difficult or challenging situations. Leads the development of patient focused education, including training to self-manage ongoing consequences of treatment. Develops new skills in response to emerging knowledge and techniques. Works across professional boundaries using creative reasoning and problem solving. Instigates and manages change within a complex environment. Influences the development of others through education and training. Promotes patient and public experience feedback through local initiatives to drive innovation and change. Provides leadership in the professional practice and professional development settings Establishes collegial partnerships Shares knowledge and skills with peers and colleagues

Learning Needs Assessment

Identifies learning needs and desires of nursing personnel. Collects data and information from a variety of sources Uses current technologies to facilitate comprehensive assessment of individual and organizational needs Analyses issues, trends and supporting data to determine educational needs Involves learners and stakeholders in formulating desired educational outcomes which reflect role competence, learning and change Evaluates progress towards attainment of outcomes using reliable instruments, using systematic methods to measure relevant outcomes

What we need from you:

Bachelor's Degree in Nursing required. Post graduate qualification within specialty Master's Degree strongly preferred, or working towards a Master's degree. Nursing registration (First Level) through the Nursing & Midwifery Council (NMC) required. Extensive working as a CNS, or equivalent, ideally working within a surgical environment. Previous experience in private healthcare preferred
