Walsall Healthcare NHS Trust is pleased to be offering an opportunity for an experienced Occupational Therapist or Physiotherapist to be part of the Intermediate Care Team, managing a team of Therapy staff providing rehabilitation, supporting hospital discharges and hospital admission avoidance. We have a high commitment to providing high quality care that is patient centered. There is opportunity to gain experience and leadership in the various pathways in Intermediate Care including supporting hospital discharges and community rehabilitation, providing therapy intervention in order to prevent hospital admission or support patients in returning home. You must be a qualified Occupational Therapist or Physiotherapist with post registration experience at band 6 level. Main duties of the job

To provide a specialised therapy service in the area of Intermediate Care/Urgent Care. To undertake a leadership role within ICS, supporting the Principals and other advanced practice with staff in planning, coordinating, delivering, evaluating, enhancing and analysing the ICS Therapy Service provided to the Trust, on a day-to-day basis.

To take a lead role in the assessment and treatment of patients, within the designated specialist area of ICS, which include those with highly complex and/or chronic presentations. To contribute towards the overall management of patients by providing treatment as indicated, and to maintain records as an autonomous practitioner.

To undertake evidence based audits to further own and team's clinical practice. To recommend changes which may lead to implementing specific alterations to practice or contribute to amended service protocols.

To participate in clinical pathway development and implementation.

To supervise, educate and assess the performance of therapy students in aspects of ICS.

To provide advice to other professionals including those from external agencies on ICS planned and unplanned practice.

Job responsibilities

In line with the Trusts policy, consent must be gained prior to any patient intervention and documented appropriately.

To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patients under the remit of the post in Community and support more junior staff and peers to do likewise.

To accept clinical responsibility for a defined caseload of patients within ICS and to organise this effectively and efficiently with regard to clinical priorities and use of time.

To interpret and analyse clinical and non-clinical facts to form accurate assessments and outcomes in a wide range of highly complex conditions within Community, and to recommend the best course of intervention, and to develop with the multi-disciplinary team comprehensive rehab plans.

To undertake a comprehensive assessment, including those with a highly complex presentation, using investigative and analytical skills, and to formulate individualised management and treatment plans, using clinical reasoning, and utilising a wide range of treatment skills and options to formulate a programme of care.

To take a lead role in the organization of group sessions.

To communicate effectively and sensitively with patients, relatives and carers to maximise rehabilitation potential, to ensure understanding of the condition and, where appropriate, to motivate and provide reassurance.

To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals. To promote understanding of the aims of therapy for patients, and to ensure a consistent approach to patient care.

To effectively communicate with patients, family / carers who may have barriers to understanding or communicating, blind, deaf, dysphasia, or who do not have English as their first language.

To communicate relevant patients clinical information effectively and to work collaboratively with all members of the multi-disciplinary team both trust wide and externally to ensure delivery of a co-ordinated multidisciplinary service. This will include case conferences, ward rounds and assistance with the discharge planning process.

To provide advice to therapy colleagues working within other clinical areas, pertaining to patient care, this includes responding immediately to queries and to be prepared patients immediately outside normal working area.

To provide patient advice, teaching and training to other members of the multi-disciplinary team. Also to provide education and training to family members and carers on specific orthopaedic individualised treatment techniques.

To prioritise own workload when necessary be responsible for adapting and delegating work responsibilities of others in the team, to meet service needs.

To maintain accurate, comprehensive and up-to-date documentation, in line with legal and departmental requirements, and communicate assessment and treatment results to the appropriate disciplines in the form of written and verbal reports.

To deal sensitively with patients who have levels of anxiety and aggression caused by pain, dementia, disability ( deaf or blind) or limited mobility and manage in a professional manner exposure to any emotional and distressing circumstances.

To attend external, local and regional meetings, as directed.

To monitor, control and prioritise the use of resources and stock available.
