£49,000 to £59,000 (dependent on experience)

Hours:

Grafton Recruitment are looking to recruit an enthusiastic and driven Lead Complex Case Manager for our client based in Birmingham.


Main Duties of the Lead Complex Case Manager:

  • Responsible for the co-ordination of specialist assessments and treatment of clients suffering acquired brain injuries.
  • Work as an interdisciplinary member of the Continuing Healthcare Team working in conjunction with other providers, both statutory and independent.
  • Develop individual pathways of care, sourcing new and existing services to meet the needs of individuals with learning disabilities.
  • To contribute to the future planning of service needs liaising with service users, user groups, providers, and commissioners, to implement service improvements for people with learning disabilities across all aspects of the client journey in the primaryand secondary health care setting.
  • Supporting and giving advice to clients and carers.

Role Criteria:

  • Registration with the Nursing and Midwifery Council.
  • Demonstratable experience of working in a large complex organisation.
  • Masters or equivalent level of experience of working at a senior level in a specialist area.
  • Working knowledge of DOLS.
  • Understanding of safeguarding issues and policy framework.
  • Excellent report writing skills, demonstrating sound clinical knowledge.
  • Experience of managing financial budgets and financial control.
  • Ability to manage competing priorities in a busy and changing environment.
  • Ability to demonstrate sensitive management of client and family/carer expectations.
