Community Practitioner working with High Risk/High Impact Perpetrators of Domestic Violence:

In partnership with Police and Crime Commission an exciting opportunity has arisen to provide specialist mental health support for the Northumbria Hub & spoke Model as a means of overcoming the barriers to engagement created by the mental health support needs of individual service users.


This post is intended to make positive impact by delivering assessment, signposting and specialist mental health support within six delivery partners who are commissioned to deliver 1:1 intensive case management for high risk, high harm and serial perpetrators of domestic abuse in the Northumbria Police force area.


This is an opportunity to work within a unique service, working across the footprint of Northumberland and Tyne and Wear.


The role involves working with established domestic violence intervention services across the six local authority areas as part of an existing MDT.


The post holder will be supported by the existing management structure of Cumbria, Northumberland Tyne and Wear NHS Liaison and Diversion service.


To provide holistic assessment of adults who are high risk, high harm and serial perpetrators of domestic abuse in the Northumbria Police force area.


To ensure that all referrals, assessments, interventions and care delivery programmes are evidence based and in line with Trust guidelines, policies and best practice documents.


To ensure effective links with relevant partnership agencies and complementary support services, and to ensure that people access these services when required.


To act as Lead Professional with limited, small case load offering a range of psych-social interventions for identified mental health needs.

To provide professional and clinical direction to members of the MDT.

To work autonomously, with the ability to organise and manage own time according to delegated workload

  • Be an accountable professional acting in the best interests of people, putting them first and providing complex clinical care that is evidence-based, person-centred, safe and compassionate.
  • Ensure evidence based practice and up to date legislation is such as local and national quality standards and frameworks are used to inform, plan and provide enhanced patient-centred clinical care.
  • Use existing knowledge and expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments.
  • Work with clinicians to ensure effective risk assessments, safety planning and direction around working with perpetrators.
  • Ensure that the relevant Safeguarding policies are implemented, embedded and governed.
  • Act independently to plan, deliver, monitor and evaluate complex care using enhanced clinical assessments, diagnostics, and interventions.
  • Contribute to the development of evidence informed risk assessment and care planning tools and resources.
  • Act as an expert resource within their own organisation and for external agencies.
  • Communicate effectively in challenging environments and situations with patients, their families/carers and the multi-disciplinary team.
  • Promote and encourage innovative clinical practice to support a culture of excellence within the wider MDT.
  • Lead and manage unpredictable and unplanned clinical situations.
