Homeless Pathway Officer

Claremont Living are providing a team of Homeless Pathway Officers who will work across Heartlands, Good Hope, Queen Elizabeth and City Road hospital sites.
Working flexibly as part of a team to deliver a 7-day service, you commit to delivering end to end coordination in support of the Discharge to Assess Model.

Key Responsibilities:
- To develop and implement effective referral systems and processes for hospital staff, to ensure that citizens have access to the service from the point they are within a hospital site.
- Liaising closely with the clinically led homeless team and your support and intervention, will enhance the out of hospital care model, helping rough sleepers to break the cycle of repeated admissions into hospital and supporting individuals towards living more settled lives.
- Demonstrating a strengthsbased approach when carrying out needs and risk assessments and show an active commitment to show an equality of opportunity for all.
- Forging strong partnerships with commissioned and noncommissioned services and providers across support and housing pathways, coordinating joint working between housing, mental health, primary care, and substance misuse services, linking individuals to the right service at the right time.
- Representing both the service and clients confidently and professionally at multiagency meetings, to support appropriate transfer of care form hospital into supported housing settings, ensuring wrap around support is person centred and meets individual needs.
- To prepare reports when required, to contribute to Key Performance Indicators, appraise outcomes and provide accurate progress reports for key stakeholders.
About You
- A current working knowledge of homeless legislation and the Birmingham Homeless Prevention Strategy 2017+
- Lived experience preferred
- 5 GCSE's or equivalent, including Maths and English
- Experience of working with a wide range of vulnerable clients
- Experience of liaison with agencies/ partner organisations
- Computer literacy including Word, Excel, Outlook and a commitment to utilising knowledge of Housing Management systems
- Ability to manage competing priorities and work to deadlines
Salary:
£22,108.00 per year with 20 days annual leave, plus 8 Bank Holidays
