To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.


The post holder will have Current Effective Status on the Nursing and Midwifery Council (NMC) - Registered Nurse (RN12) or Paramedic (HCPC.

To be responsible, and accountable, for service delivery to clients/patients.


To be able to assess and develop plans of care to meet the complex needs of patients with a variety of conditions.

This includes chronic, acute and palliative care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes.

To be able to initiate referrals to other health professional specialist services and agencies.


To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life.

To ensure practice is supported by research, evidence-based practice, literature and peer review.

COVID-19 Vaccination

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.

All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.

The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London


This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).


The post holder will be a Registered Nurse (RN12-NMC) or Paramedic (HCPC) who will have significant intermediate care experience at Band 5 or 6.

Must possess Clinical Assessment Skills, Minor Illness and either Non-Medical Prescriber or working towards qualifications. The post holder will demonstrate safe clinical decision making and expert care for the patients registered within the service.

The post holder will be professionally and legally accountable for all aspects of care from initial history taking through to treatment.

They will be responsible for patients allocated to them and will assess, manage and deliver care to meet the complex needs for those patients requiring intermediate care and primary care services.

The post holder will be able to manage and support their peers, working on their own initiative and but also an effective team member along with the requirements of pre-registration students.

They will be committed to working collaboratively, actively.
