Job summary

An exciting opportunity has arisen for a Clinical Nurse Specialist / Clinical Practitioner to join our team at Mountbatten.

Working at an advanced practice level, you will completecomprehensive, holistic assessments including developing individualisedpalliative and end of life care and treatment plans, using advanced careplanning skills, in the community and outpatient settings across the island.

You will manage your own caseload, ensuring that you providetotally holistic care to the patient and their family.

Main duties of the job

Responsible for managing your own caseload of patients andfor supervising junior members of the nursing and care at home teams asrequired, using recognised referral guidelines and agreed levels of nursing intervention. You will incorporate a multi-professionalapproach across all health, education and social care settings to enable thehighest standards of care to be provided to your patients, their families andcarers.

Job description

Job responsibilities

The post holder will work autonomously and as part of a teamcontributing to the provision of 24/7 Specialist Palliative and End of LifeCare, providing an environment that encourages patients and their families toplay an active part in their care.

The successful candidate will provide advanced levels of clinical decision-makingthrough assessment and interpretation of clinical information to supportpatients, families and carers who have complex or rapidly changing physicalsymptoms, emotional or practical needs.

There is a requirement to work days,nights and weekends on a rota basis - shift allowance given for weekend,evening and bank holiday working.

Essential

Skills in negotiating, problem solving and report writing. Excellent influencing and interpersonal skills with people at all levels, internally and externally Leadership skills and management experience at Band 6 or above. Can inspire others by leadership behaviours

Qualifications

Essential

First Level Registration with NMC and in possession of a relevant degree in Health/Nursing Studies or recognised Higher Education Paramedic qualification Registered Independent Prescriber (V300) or willingness to undertake Evidence of continued professional development Master Level qualification/willing to work towards Master Level

Abilities

Essential

Understanding palliative care and EOLC National agendas Ability to lead and influence change In depth knowledge of specialist symptom management and advanced practice

Other

Essential

Current Driving Licence Use of a vehicle, with insurance for business purposes Demonstrates a commitment to continuous professional development

Experience

Essential

Experience of working collaboratively within multidisciplinary teams and the ability to form good working relationships at all levels including volunteers Experience of dealing with vulnerable adults in a care setting Experience of working in a hospice or providing EOL within a community care setting Relevant post registration experience, at Band 6 or above, in cancer, palliative care or the speciality Leadership and management experience within a Hospital/Hospice or Care setting
