Do you have a particular interest in Urgent Community Response? We are looking to enhance our ability to deliver Urgent Community Response within our North and West Bristol Locality.


This is an exciting opportunity for a registered Nursing Associate or Assistant Practitioner to work as part of multi-disciplinary integrated network teams with a focus on delivering our urgent care service.


By joining us you will have many opportunities to develop your clinical skills as well as physical assessment and clinical reasoning with the support of Advanced Clinical Practitioners and wider members of the team.


You will work as a member of the Urgent Care Team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.


You will:

Maintain a high professional standard of care in adherence to all protocols/policies & guidelines

Respond to urgent requests for assessment from patients known to the Integrated Neighborhood Team Assess

Manage a patients care plan within your scope of practice and with the support of the wider Urgent Care team

Provide clinical assistance to other members of the clinical team

Maintain accurate electronic clinical records

You will work within our service that is delivered from 8am-8pm, 7 days a week service.


You will be required to respond to urgent referrals in need of complex management and follow predetermined management plans to both prevent hospital admissions and to facilitate early discharges.


You will work as part of the Urgent Care team to proactively monitor and manage patients on the caseload to ensure the best outcomes for them.
